Before services are performed, you will be asked to read and sign our liability waiver. To facilitate a smoother chick-in process, we are providing you with the contents of the waiver in advance, allowing you to review it at your convenience.

Liability Waiver
Please read carefully. By signing this waiver, you acknowledge and accept the risks and terms below.
Your pet is important to us, and every effort will be made to make your pets visit as safe and pleasant as possible.
I understand that there are inherent risk associated with pet grooming, including, but not limited to cuts, nicks, scratches, clipper irritation, toenail bleeding, reaction to products and physical stress. Grooming may occasionally expose a hidden medical condition or aggravate an existing one. Senior pets, puppies, and pets with severe matting or behavioral challenges are at higher risk of injury or stress. I certify that I am the legal owner or authorized guardian of this pet. In the event that I am not present, I authorize Sudz of Luv Dog Grooming to seek emergency veterinary care for my pet at my expense, if deemed necessary.
I acknowledge that interactions between pets and humans may result in injury or property damage. This includes, but is not limited to, scratches, bites, personal disability, illness, or injury to humans or other pets. I understand my pet may need to be restrained for safety purposes, and that in doing so, additional injury may occur. I accept all risks and costs, including any veterinary expenses, resulting from participation in grooming services and release Sudz of Luv Dog Grooming from all liability.

Matting Disclosure:
I understand that pets with severely matted coats are at higher risk for skin irritation, clipper burns, bruising, and other complications. Removing mats can cause discomfort and may reveal underlying skin issues. I release Sudz of Luv Dog Grooming from all liability related to the grooming of matted coats.

Parasites:
If fleas or ticks are found, Sudz of Luv Dog Grooming may apply treatment shampoo at my expense. I release them from responsibility for any adverse reactions or negative effects that may arise from the application of the product.
Property Release:
By signing below, I release the property owner(s) of 1431 Keokuk St., Hamilton, IL 62341 and Sudz of Luv Dog Grooming from any and all claims related to personal injury, property damage, or loss that may occur from my presence on the premises. I assume full responsibility for myself, my pet, and any resulting damages or injuries.
